FREE dental clinic provided by Onsite Dental at your student's school on the following dates:

 

 

Please complete the dental consent form with a parent/guardian signature and return to your student's school nurse to receive the FREE dental services. 

 

Your student will receive an exam, cleaning, fluoride treatment, sealants (a protective coating on the chewing surfaces of the back teeth), and dental education from a licensed dentist and hygienist.  

 

The State of Illinois requires students in Kindergarten, 2nd, 6th and 9th grades to provide proof of a dental examination to the school by May 15, 2024.
