Physical & Immunization Requirements
To help ensure the health and safety of all students and comply with Illinois law, Freeport School District 145 reminds families that students entering Kindergarten, 6th grade, 9th grade, or an Illinois school for the first time (regardless of grade) must have a completed physical examination and required immunizations on file with the school.
Under Illinois School Code (105 ILCS 5/27-8.1), proof of the required health examination and immunizations must be submitted to the school by October 15, 2026. Parents/guardians should ensure all required health forms, including the student's health history, are completed and submitted by this deadline.

DENTAL & VISION EXAMS
The State of Illinois requires all students in Kindergarten, 2nd and 6th grades to provide proof of a dental examination to the school by May 15, 2027. All Illinois children upon first entry into school, including all Kindergarten students, must also provide proof of an eye exam performed by an ophthalmologist or an optometrist by October 15, 2026.
